Immunization Requirements
To protect the health of all BOUSD students and staff, and to curtail the spread of infectious diseases, the following are the required immunizations for school entry. California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry.
- Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap) - Five doses
- Four doses OK if one was given on or after 4th birthday.
- Booster: For 7th grade advancement, 1 dose is required on or after 7th birthday.
- Polio - Four doses at any age OR three doses is OK if one was given on or after 4th birthday
- Hepatitis B - Three doses
- Measles, Mumps, and Rubella (MMR) - Two doses
- Varicella (chickenpox) - Two doses

Required Immunizations for Enrollment in Public School
California law requires all students enrolling in Transitional Kindergarten (TK) and Kindergarten to be current on state-mandated immunizations before attending school. Parents/guardians must provide proof of immunization records as part of the enrollment process.
If a child has not yet received all required immunizations due to age-related scheduling, parents/guardians should still complete and submit enrollment paperwork. Documentation of future immunization appointments or a timeline for completion may be required. Students must have all required immunizations before the first day of school unless they have a valid exemption per state law.
Acceptable forms of vaccine documentation include a California yellow immunization card, official medical record, California immunization registry printout, or record from a previous school. Unfortunately, we cannot accept handwritten documents without an official medical office stamp or illegible documents.
